Overview
#

A vulnerable item should not be treated as a static finding. It should be reviewed as a piece of operational security work that needs risk context, ownership, remediation direction, validation, and clear documentation.

This checklist organizes the review process into practical triage stages.


1. Vulnerable Item Intake
#

Initial review should confirm what entered the workflow and whether the finding has enough context to be actionable.

Review questions:

  • What vulnerability or finding was identified?
  • What asset, CI, host, application, or service is affected?
  • Is the finding tied to a known CVE or vulnerability entry?
  • Is severity or risk rating available?
  • Is scan/source information available?
  • Is this a duplicate, reopened, or newly observed finding?
  • Is the affected asset still active and relevant?

2. Risk and Priority Review
#

Triage should go beyond raw severity. A high-severity finding on a non-critical asset may not carry the same operational priority as a medium-severity finding on an exposed critical system.

Review questions:

  • What is the technical severity?
  • Is exploitability known or likely?
  • Is the asset internet-facing or externally reachable?
  • Is the affected CI business-critical?
  • Is there known active exploitation?
  • Is the vulnerability tied to ransomware, privilege escalation, remote code execution, or lateral movement risk?
  • Does the asset support an important business or operational function?
  • Are compensating controls already in place?

3. Ownership Assignment
#

A vulnerability without clear ownership often becomes unresolved risk. Assignment should be based on asset ownership, service responsibility, support group mapping, or remediation accountability.

Review questions:

  • Which team owns the affected CI or service?
  • Is there a support group mapped to the CI?
  • Is the assignment group accurate?
  • Is the assigned team responsible for remediation or only review?
  • Is escalation needed because ownership is unclear?
  • Is the remediation owner different from the business owner?
  • Is there enough information for the owner to act?

4. Remediation Path
#

The triage process should identify what action is expected and whether a remediation task, change, patching activity, configuration update, or exception path is needed.

Review questions:

  • Is a patch, upgrade, configuration change, or compensating control required?
  • Is remediation feasible immediately?
  • Does the fix require a maintenance window?
  • Is change approval required?
  • Is the remediation action clear enough for the assigned team?
  • Is there a remediation deadline or SLA?
  • Should a remediation task be created or updated?
  • Are implementation notes documented?

5. Exception and False Positive Review
#

Not every finding can be remediated immediately. Some require risk acceptance, compensating controls, vendor review, or false-positive validation.

Review questions:

  • Is the finding a confirmed vulnerability?
  • Could this be a false positive?
  • Is the vulnerable software actually installed or reachable?
  • Is the affected asset out of scope, retired, or decommissioned?
  • Is remediation blocked by business, vendor, compatibility, or operational constraints?
  • Is risk acceptance required?
  • Are compensating controls documented?
  • Is an expiration/review date required for the exception?

6. Validation and Closure
#

Closure should be evidence-based. A vulnerable item should not be closed only because someone says remediation happened.

Review questions:

  • Was remediation completed?
  • Was the finding rescanned or otherwise validated?
  • Is there evidence of patching, configuration change, mitigation, or risk acceptance?
  • Is the final state documented?
  • Was the correct closure reason selected?
  • Are notes clear enough for audit or later review?
  • If exception-based, is the review date documented?
  • If false positive, is the reasoning documented?
